Step 1:

Download the Confidential Estate Planning Questionnaire. It is is PDF form. (If you have a PC, we suggest you "right click" and select "Save Target As" to save to your computer)

Step 2:

Fill out the Questionnaire. It is a fillable pdf, so you can fill it out online and then print it. . . or you may print it and then fill it out by hand. Please PRINT CLEARLY.

Step 3:

FAX the completeld Questionnaire and signed Fee Agreement to my office at (480) 275-5626.

Step 4:

Call. As soon as we receive your Questionnaire, we will set up a telephone meeting with Mr. Smith to discuss your specific needs and requirements. Call our office to set up your appointment (480) 807-9300.

As soon as we receive payment, we will begin work on your Living Trust. You can expect your Living Trust to be mailed in approximately three weeks.
